Serving Southern California since 2010, It has been our assertion that incorporating a comprehensive standardized assessment into a care plan is insufficient. We believe that clinical judgment, creativity, sensitivity, and evaluation over time must be in place to provide patient-client holistic care.
Personal preference along with standing medical directives are obtained and analyzed in our assessment process. From there, we identify with the patient-client and family members the areas of concern, desired outcome, and services to pursue in support of clinical and functional goals.
